FAQS related to Claim Form - Part A and Part B

When an insured is opting for reimbursement claim, they have to fill up the claim form. While Part B of the claim form is not mandatory to fill up in case of raising a claim for pre and post hospitalization expenses, Part A of the claim form is mandatory to be filled while raising a reimbursement claim. 

There exists some ambiguous components in the claim form which varies from insurer to insurer, that's why, it's crucial to fill up the claim form correctly to avoid delay in your claim initiation process. 

Some frequently asked queries are as follows: 

How do I fill the claim form? Digitally or Manually?  
First of all, you can download the claim form from the portal itself, if you don't know how to then, please follow this guide. Once the claim form has been downloaded you can make use of several apps available for digitally filling up the form such as Adobe Acrobat, Foxit PDF Editor etc. You can also manually fill out the form by taking a print-out of the same.

What should I fill in place of SI No./ Certificate No.? 
SI No./Certificate No. is not mandatory to be filled so, you can leave it blank. 

What should I fill in place of Company ID/ TPA ID (MA ID) No.? 
Each insurer use different terminologies for this ambiguous component of the claim form. The insured has to fill their (or the patient's) Member ID or Client ID (whichever is mentioned on the e-card). What format is supported for uploading the claim form on the Nova Portal/App? 
You can upload any format such as PNG, JPG, JPEG etc. as long as the image uploaded is clear and the components of the claim form are easily understandable (if filled manually). 

Should I fill Part B of the claim form while going for pre and post hospitalisation expenses? 
An insured is not obligated to fill Part B of the claim form while raising a claim for pre and post hospitalisation expenses. However, while raising such a claim do keep in mind the time frame stated in your policy document to claim for these expenses. My hospitalization was done in a different location than my current one and it's too far away from my current location, how should I go about filling the claim form Part B in this case? 
The general of thumb here is that when you get discharged from a hospital, you should collect all the documents which will be required while raising a reimbursement claim, along with, getting the Part B of the claim form filled from the hospital's side. However, in such a case, kindly contact your respective claims associate and inform them about this situation.
