Where can I find claim form, i.e., Part A & B form?

Claim form is required to be filled when the insured is opting to go for a reimbursement claim. When you have been discharged from the hospital, the hospitalization expenses which were paid out of your pocket can be covered by raising a reimbursement claim (T&C applied). 

  1. Once you login into the portal, click on guide to reimbursement claims on the homepage.
  1. Upon opening the guide to reimbursement claims, there's a hyperlink attached on the "claim form" which is unique to every individual depending on who is the TPA for the insured. 


  1. You can now download the claim forms once you click on "here" which is a hyperlink attached, unique to every individual.
