Where can I find claim form, i.e., Part A & B form?
Claim form is required to be filled when the insured is opting to go for a reimbursement claim. When you have been discharged from the hospital, the hospitalization expenses which were paid out of your pocket can be covered by raising a reimbursement claim (T&C applied).
- Once you login into the portal, click on guide to reimbursement claims on the homepage.
- Upon opening the guide to reimbursement claims, there's a hyperlink attached on the "claim form" which is unique to every individual depending on who is the TPA for the insured.
- You can now download the claim forms once you click on "here" which is a hyperlink attached, unique to every individual.
