What is my Company/ TPA ID/Member ID in the claim form?

There are certain components in the claim form which might be confusing to fill up without proper knowledge in the subject. That's why, it's important to fill up the form correctly to avoid any delay in the initiation of your claim process. 

To find out what is your TPA ID, kindly download your e-card from the Nova Portal. If you don't know how to, then please read this guide. If your e-card isn't available on the Nova Portal, then please read this guide. 

  1. For employees with IFFCO Tokio as their insurer, kindly keep these in mind while filling up the form: 



  1. As you can see in the image above that it is written as company/TPA ID No. in the claim form, what it means is that the insured has to fill their Member ID here as mentioned in their e-card. The terminologies may vary from insurer to insurer, the simple way to understand one's member ID is that it will always be unique for every individual, even for your dependents. 


  1. For employees with Care Health Insurance as their insurer, kindly keep these in mind: 
 


  1. As you can see in the image above that it is written as company/TPA ID No. in the claim form, what it means is that the insured has to fill their Client ID here as mentioned in their e-card. The terminologies may vary from insurer to insurer, the simple way to understand one's Member ID/ Client ID is that it will always be unique for every individual, even for your dependents. 



  1. For employees with the New India Assurance Co. Ltd. (Medi Assist) as their insurer, kindly keep these in mind: 



  1. As you can see in the image above that it is written as company/TPA ID (MA ID) No. in the claim form, what it means is that the insured has to fill their Member ID here as mentioned in their e-card. The terminologies may vary from insurer to insurer, the simple way to understand one's Member ID is that it will always be unique for every individual, even for your dependents. 



  1. Kindly note that serial/certificate no. is not required to be filled in case of any insurer while filling up the claim form.
